How much are contacts with insurance?
With vision insurance, contacts cost you the price of your lenses minus a fixed allowance, plus a fitting fee. The four 2026 federal employee (FEDVIP) vision plans pay $120 to $170 toward contacts once a calendar year, and only instead of glasses, not on top of them. Anything above the allowance is yours to pay.
So the real question is the size of your lens bill. A year of daily lenses can cost several times the allowance.
How does a contact lens allowance work?
The plan gives you a dollar amount for contacts, not a set number of boxes. Three rules to know:
- Contacts or glasses, not both. The allowance is usually "instead of glasses." Aetna's 2026 FEDVIP brochure says it plainly: "You cannot use both benefits within the same calendar year."
- It resets on a schedule. All four FEDVIP plans reset every calendar year. Check your own plan's frequency.
- Some plans want it used at once. BCBS FEP Vision's 2026 allowance "must be used all at one time."

The exam and the fitting are billed separately
The contact lens fitting and evaluation is a separate service from the routine eye exam, and plans cover the two differently.
On VSP's 2026 FEDVIP plans, the exam is a $10 copay, or $0 at Premier Edge locations. The contact lens exam is a separate copay of up to $55. Aetna, BCBS FEP Vision and UnitedHealthcare charge nothing for the in-network routine exam, but each bills the fitting its own way.
What you pay once the allowance runs out
Everything above the allowance. A worked example:
- Eight MyDay 90-packs is a year of daily lenses for both eyes. On Eyeconic, VSP's online store, the annual supply price was $106.19 a box on September 28, 2026. That is $849.52 for the year.
- Take off VSP High's $150 allowance. You pay $699.52 for lenses.
- Add the contact lens exam copay of up to $55 and the $10 exam copay.
Some plans cut the overage a little. Aetna gives 15% off the cost over the allowance on conventional lenses. BCBS FEP Vision gives 15% off the overage at in-network independent providers, but not at national or online retailers.

In-network vs out-of-network
Out of network, you pay in full and file a claim for a fixed amount back, usually less than in-network. For elective contacts in 2026:
- VSP: up to $105, on both options.
- Aetna Vision Preferred: up to $120 Standard, up to $136 High.
- BCBS FEP Vision: up to $75 High. Standard pays nothing out of network.
- UnitedHealthcare Vision: up to $125.
Medically necessary contacts
Plans cover "necessary" contacts under separate rules. The brochures list conditions such as keratoconus, anisometropia and high ametropia. Each plan sets its own criteria.
- VSP: $20 copay Standard, $10 High.
- Aetna Vision Preferred: nothing in network. Up to $210 back out of network.
- BCBS FEP Vision: covered up to $600. Pre-authorization may be required.
- UnitedHealthcare Vision: $25 copay Standard, $0 High. Your provider must contact the plan before you buy.
Stacking the allowance with rebates and FSA money
Rebates. A manufacturer rebate can sit on top of your allowance. CooperVision's 2026 rebate pays $100 back on a year of MyDay, only when you buy from the eye doctor who prescribed the lenses. Internet retailers are excluded. Its terms also say that an insurance claim "must be based upon your payment less the amount of the rebate." More in how do contact lens rebates work.
FSA and HSA. The part the allowance does not cover can often go on an FSA or HSA. IRS Publication 502 counts "contact lenses needed for medical reasons" as a medical expense, and your plan sets the rules. For 2026, the health FSA limit is $3,400, with up to $680 carryover if your employer allows it. Details in can you use FSA or HSA for contacts.
Your prescription is yours to take elsewhere
Under the federal Contact Lens Rule (16 CFR 315), the prescriber must give you a copy of your prescription after the fitting, whether or not you ask. They cannot make you buy lenses first, charge an extra fee for the copy, or make you sign a waiver. A contact lens prescription generally lasts at least one year, unless your doctor sets a shorter date for a medical reason. How ordering elsewhere works is in buying contacts online.
